Ukrainian citizen passport or id-card, with annex №13;
Identification code;
If the person was previously married (Certificate of divorce; court decision on divorce, death certificate of one of the spouses)
If the person was previously married, an additional document confirming the termination of the previous marriage or annulment of marriage (Certificate of divorce issued in the name of the customer; court decision on divorce, annulment of marriage, which entered into force death certificate of one of the spouses)

Passport of a foreigner and a certified copy with a translation into Ukrainian;
If a foreigner has a residence permit issued in Ukraine you will also have to add it;
Please note! State Migration Service of Ukraine is aloud to check the legality of the foreigner’s stay on the territory of Ukraine. This procedure is obligatory and will take for a couples of days.
If the person has already been married, a certified copy of the document confirming the dissolution of the previous marriage, legalized (apostilled) in the prescribed manner with a certified translation of all pages into Ukrainian, is additionally provided.
If the bride (or one of them) is not a citizen of Ukraine:

– Submission of an application 24 hours before the registration of marriage. (According to the legislation of Ukraine, foreigners before the state registration of marriage have the right to submit documents to the Organizer to verify the legality of a foreigner’s stay in Ukraine.

Form of submission of documents
